Canadian Cardiovascular Society Post-ACS Secondary Prevention Study
The Canadian Cardiovascular Society (CCS) has developed a secondary prevention pathway for post-ACS patients. This pathway, created by a multidisciplinary team, aims to ensure evidence-based care at three phases: (1) Hospital discharge, (2) Early follow-up (1-30 days post-discharge) and (3) Late follow-up (>30 days post-discharge). CEBI is assisting with the development of clinician surveys to identify needs and barriers to implementing the CCS pathway, and will use feedback from stakeholders to revise and finalize the pathway, for dissemination.
