Director: Dr. JD Schwalm
Program Overview
First established in 2016 as the Centre for Evidence‑Based Implementation (CEBI), the program became the Knowledge Mobilization stream of the HHS Knowledge Centre in 2025. Its purpose is to ensure that evidence‑based recommendations are effectively translated into routine clinical practice across HHS.
Core Activities
The program identifies effective strategies for implementing proven best practices and ensuring their long‑term sustainability. Knowledge mobilization efforts focus on bridging the gap between published evidence and real‑world clinical application, particularly at the point of care.
Capacity Building & Collaboration
Multidisciplinary collaboration is central to the program’s approach. Teams across HHS receive support to address clinical challenges and improve patient outcomes, patient experience, processes of care, and system performance. The program engages a broad range of clinical areas, including cardiovascular care, oncology, immunology, infectious diseases, trauma and critical care, children’s and women’s health, rehabilitation medicine, and aging.
Impact & Institutional Contribution
Through its implementation science expertise, the program advances healthcare delivery across the organization. Its work strengthens HHS’s ability to adopt evidence‑based practices consistently and sustainably, contributing to improved care quality and system performance.
